For anyone else struggling with this, what helped me the most was sprinkling motivational/uplifting music into my music playlist. It’s a constant reminder to work on my mental health and helps with the consistency aspect thats so hard to keep up with otherwise. Fearless Motivation makes some good motivational songs.
You don’t need to do the work on your own. Just be brave enough one day to ask for help. It all gets better after that.
