TLDR: We imagine death comes in threes because we think of things in threes. In fact, people die all the time, and some of them are notable, and we choose which ones go together.
I talk about my cousin who died because he was in the grandstands at the Reno Air Races when one of the racing planes lost control and careened into the grandstands, a particularly unusual way to die.
His family is super religious, and they were desperate to understand why, how this fulfilled a divine plan. In the end, it comes down to shit happening. Attendance at air races isn’t even particularly risky, but sometimes the dice role a fumble.
